Qualitative Feedback form

On the bottom of the Account page on Picplum. Considering putting it on the bottom of all pages. Inspired by Kissmetrics and Stripe who have had such forms on the bottom of their pages. I want to change the question and textarea default microcopy though. Any suggestions?
