Performance and Motivation Trend (Dashboard Card)

Hi guys! :)

The idea behind this is to showcase managers a trend on the performance and motivation of their direct reports. Performance is rated by the managers and motivation level input is given by their report. This is collected almost every week. To ensure that the manager rates it consistently, the trend is hidden until they don't give a rating. So, the performance check-in acts as a gateway to view trends.
