001 DailyUI - Sign Up

While researching some UI inspirational and educational material, I found DailyUI, which is a daily email prompt for UI designs. The assignment was:

"Create a sign up page, modal, form, or app screen related to signing up for something. It could be for a volunteer event, contest registration, a giveaway, or anything you can image."

Here's to the first one. I would love some constructive feedback, as I dive more into the world of UI.
